How health-related information about you may be used and disclosed, and how you can get access to that information. Please review it carefully.

Our commitment
Arranging medical transportation means we necessarily learn things about your health — where you are being treated, how you move, what equipment you use. We are committed to protecting that information and to using it only as described here or as permitted by law.
[CONFIRM WITH COUNSEL whether Universal Healthcare Logistics is a covered entity or a business associate under HIPAA for the services it provides. The scope of this notice depends on that determination.]
How we may use and disclose your information
- For treatment-related coordination — sharing pickup details and mobility needs with the assigned crew, and coordinating timing with your clinic, dialysis center or discharging hospital.
- For payment — billing your health plan or payer for a covered trip.
- For our operations — scheduling, quality review, staff training and safety investigations.
- As required by law — including public health, safety and law enforcement requirements, and in response to a valid court order.
- To people involved in your care — a family member, caregiver or personal representative involved in arranging your transportation, unless you object.
Any other use or disclosure requires your written authorization, which you may revoke at any time in writing.
