The terms that apply when you book non-emergency medical transportation with us or use this website.

Agreement
By booking a ride or using this website you agree to these terms. If you are booking on behalf of another person, you confirm you are authorized to do so and to share the information needed to arrange their trip.
What this service is — and is not
We provide non-emergency medical transportation only. We are not an ambulance service. We do not provide emergency response, and our crews do not administer medical treatment during a trip.
If you are experiencing a medical emergency, call 911.
Booking and confirmation
A trip is scheduled only once we confirm it. Confirmation includes the pickup window, the vehicle type, the level of assistance and the rate. Availability is limited and same-day requests cannot be guaranteed. Wheelchair and stretcher trips require more lead time than ambulatory trips because of crew and equipment requirements.
Accurate information
We assign the vehicle and crew based on what you tell us. Please give us accurate details about mobility level, equipment, weight where relevant, oxygen requirements and building access. If the information changes, tell us before the pickup. We may be unable to complete a trip safely if conditions differ materially from what was described, and a cancellation fee may apply in that case.
Pricing and payment
Rates depend on mobility level, distance and time of day, and are quoted before the trip. Payment is due [PAYMENT TERMS]. Where a trip is billed to a health plan, you remain responsible for any amount the plan does not cover.
Cancellations and no-shows
Please cancel as early as you can so the slot can go to another passenger. Cancellations with less than [NUMBER] hours’ notice, and no-shows, may be charged at [AMOUNT]. We waive this for genuine medical circumstances.
Timing
We build pickup windows around your appointment with traffic accounted for and we plan to arrive early. We cannot guarantee exact arrival times, and we are not liable for missed appointments caused by traffic, weather, road closures or other conditions outside our control.
Conduct and safety
Passengers and companions must follow the crew’s safety instructions, including seat-belt and wheelchair securement requirements. We may end a trip where behaviour puts the passenger, the crew or the public at risk. Smoking, alcohol and illegal substances are not permitted in our vehicles.
Companions and personal property
One companion may travel at no additional charge where seating allows; tell us when booking. Please keep personal belongings with you. We are not responsible for items left in a vehicle, though we will make reasonable efforts to return them.
Limitation of liability
To the fullest extent permitted by Florida law, our liability arising out of a trip is limited to the amount paid for that trip. We are not liable for indirect or consequential losses. Nothing in these terms limits liability that cannot be limited by law.

Governing law
These terms are governed by the laws of the State of Florida, and any dispute will be handled in the courts of Miami-Dade County, Florida.
